I have seen similar requests but none using clock prompts. This is a compliance issue

@TimePays client Jared Auerbach wants to create a prompt with checkboxes to answer 5 Covid 19 questions that MUST be answered to allow a valid PUNCH IN. They would prefer check boxes to be checked rather than having to use 5 separate prompts. they want to use the clock to screen employees for COVID. Can we make this prompt to be a list with several questions and check boxes on it OR do we have to make 5 separate prompts?
Please advise the best way to do this and where we should start in the program to make this work?
